Furniture, Fixture, & Equipment Asset Liquidation Form

Please use this form for almost anything, except technology items. Make sure to email or fax the form to the warehouse for scheduling and pick up.

*This form is also in Eduphoria and may be submitted through Eduphoria. This is a great option for teachers to submit items for liquidation on behalf of the campus instead of leaving it all to the front office. Principals must approve the items and form in Eduphoria before it will be forwarded to the warehouse for scheduling and pick up.

Please use the Pick Up Request Form to have items picked up from your campus and/or taken to another campus.

This form may be used to pick up small items or a couple of boxes for asset liquidation - large items or amounts must be scheduled. Please make sure to include an approved asset liquidation.
