ALL ATHLETES ARE REQUIRED TO SUBMIT:

• COMPLETED MEDICAL HISTORY FORM

• COMPLETED PREPARTICIPATION PHYSICAL / MEDICAL HISTORY

• COMPLETED RANKONE ONLINE FORMS

MUST BE SUBMITTED TO A BOYS ATHLETIC COACH PRIOR TO PARTICIPATION

GCISD requires ALL students to obtain a NEW physical dated April 2024 or later for the 2024 - 2025 school year. Medical history forms and physicals will be collected by coaches on the first day of school or during our football meeting/equipment pick-up date. Any CTMS equipment will NOT be passed out without a submitted physical/medical history form.

Examples of where you can get a physical exam completed:

• OPTION 1: Physicals can be completed with your physician or pediatrician.

• OPTION 2: Total Care in Hurst. Families need to call the office and make an appointment first--no walk-ins. The address is 400 Mid-Cities Blvd in Hurst, phone 817-554-8200

• OPTION 3: EMC Express Care does school sport physicals for $10 with no appointment necessary. 817-503-8800 Open 10am-8pm every day

• OPTION 4: CVS on Pool Road, across the street from CTMS has a Minute Clinic that completes physicals quickly.
