Insurance Card Information:

• Medical/Pharmacy Card: Allegiance Customer Service at (855) 999-6808 can confirm your mailing address and reissue your health insurance card.  Allegiance a Cigna Co. is the provider and Cigna Open Access Plus is the network.

• Prescription questions:  RxBenefits + OPTUMRx at (800) 933-0765 or email Customer Care.  

• Vision:  Superior Vision at (800) 507-3800 or Superior Vision Website.

• Dental:  Cigna High or Low at (800) 244-6224 or Cigna Dental Website.  Dental offices can locate your profile without an ID Card.
