Text Box: WITHDRAWAL INFORMATION Text Box: Parent/Guardian Must: Notify the registrar at least one day in advance of their intent to withdraw the child Complete the withdrawal papers stating the reason for the request Sign the request form Student (on the last day) Must: Return all textbooks to the registrar Return all library books and pay fines (if applicable) Clear balance in cafeteria account (either pay or get money back) Turn in all athletic uniforms (if applicable) Clean out locker
